About the position

The Community Support Specialist at Compass Health Network plays a crucial role in providing integrated, holistic, community-based support services to clients dealing with mental health and substance abuse issues. This full-time position focuses on empowering clients through individualized education and support, helping them achieve their recovery and resiliency goals while ensuring access to necessary public services.

Responsibilities

• Provide individualized and group education services based on clients' strengths, needs, abilities, and preferences.
• Assist clients in identifying their strengths and needs regarding substance abuse and mental illness, and participate in the development of Wellness Plans.
• Incorporate coaching surrounding the Eight Dimensions of Wellness into service provision.
• Support clients in completing goals identified on Wellness Plans and review these plans as needed.
• Aid clients in accessing public services such as financial assistance, housing, and food.
• Maintain contact with hospitalized clients and participate in discharge planning.
• Provide education and skill-building in daily living skills, employment, symptom management, and budgeting.
• Assist clients in developing personal support systems, including family and recovery communities.
• Accompany clients to appointments to address medical or health needs.
• Follow up with clients regarding appointments and completion of necessary forms.

Requirements

• Experience in providing community-based support services.
• Knowledge of mental health and substance abuse issues.
• Ability to develop and implement Wellness Plans collaboratively with clients.
• Strong communication and interpersonal skills.
• Experience in education and skill-building related to daily living and recovery.

Nice-to-haves

• Experience working with diverse populations.
• Certification as a Qualified Mental Health Professional (QMHP).
• Familiarity with local public services and resources.
