The Community Support Specialist position at Simple Steps, LLC is a part-time role focused on enhancing the quality of life for Veterans with neurological disorders, physical disabilities, and traumatic brain injuries. This position is designed for individuals who are passionate about providing high-quality home and community services to those in need. The Community Support Specialist will work closely with Case Managers and family members to deliver dynamic social, recreational, wellness, educational, and individualized life skills training and activities. The role requires effective communication and collaboration to ensure that client care plans are executed properly and that resources are utilized efficiently to meet the needs of the clients. In this role, the Community Support Specialist will be responsible for supporting and implementing care plans, educating clients and their families about healthcare services, and maintaining accurate documentation for invoicing purposes. The position involves traveling to clients' homes, providing transportation for clients, and engaging in various activities that promote the well-being of the clients. The ideal candidate will have experience working with individuals with disabilities and will be able to manage their own time effectively while also being flexible to accommodate the needs of the clients. The Community Support Specialist will report to their assigned Care Coordinator and will be expected to track progress on SMART goals through weekly documentation.
