About the position

The Community Support Specialist at Sertoma Star Services plays a crucial role in empowering individuals with intellectual/developmental disabilities and mental illness. This full-time position involves providing support services to consumers, conducting assessments, developing treatment plans, and ensuring documentation compliance. The specialist will work primarily off-site in community settings, collaborating with various stakeholders to enhance consumer recovery and community functioning.

Responsibilities

• Provide direct services to individuals assigned to the Community Support Team (CST), primarily off-site in community settings.
• Complete all required documentation according to Sertoma policy and relevant standards.
• Conduct necessary assessments and develop treatment plans in collaboration with consumers and other stakeholders.
• Implement crisis plans and treatment goals in accordance with established standards.
• Collaborate with CMHC staff to ensure coordinated care and provide referrals as needed.
• Maintain records of individuals on the CST in compliance with policies and standards.
• Assist CST Lead in conducting quality assurance of CST records.
• Participate in weekly team meetings to discuss consumer progress and needs.
• Maintain effective communication with agency staff, families, and other providers.

Requirements

• Bachelor's Degree in human services or equivalent experience in mental health or human services.
• Minimum of one year experience working with individuals with mental health and/or substance use disorders.
• Effective communication skills, both oral and written.
• Valid driver's license and current auto insurance documentation.
• Proficient in Microsoft Office Suite and other software applications.
