The Community Support Team (CST) Case Manager plays a crucial role in providing mental health rehabilitation services to adults and families. The primary objective of this position is to decrease hospitalization and crisis episodes while enhancing community functioning, enabling clients to achieve their rehabilitative, resiliency, and recovery goals. The Case Manager is responsible for delivering program services in an ethical, legal, and moral manner within a safe and therapeutic environment, adhering to applicable regulatory and accreditation standards. As a member of the CST, the Case Manager will engage in various activities including case management services, crisis intervention, skills training, and documentation within required time frames. They will also conduct individual counseling, monitor medication, attend meetings, and participate in quality improvement initiatives. The role requires a collaborative approach to mental health services, working closely with clients and their families to facilitate recovery in their daily lives. The Case Manager must be adept at motivating and problem-solving with staff regarding effective clinical practices. They will be expected to adapt their communication style to meet the needs of clients and their families, ensuring that all necessary documentation is completed within appropriate timelines. The position also involves primary case management responsibilities for designated clients, conducting individual, group, and family counseling sessions, and maintaining client service records in compliance with regulations. Additionally, the Case Manager will coordinate services from various community resources and present educational workshops and training sessions as needed. This position is integral to the Clinical Team, participating in team meetings and activities while upholding confidentiality and delivering exceptional customer service. The Case Manager will also be familiar with court proceedings and may be required to make court appearances on behalf of clients.
