Opportunity awaits! We are hiring Complex Case Manager (Human Services Case Manager) to join our team! In this position, you will assess Oregonians applying for, or currently receiving, long-term care services to develop, coordinate, and arrange a variety of services to allow older adults and people with disabilities to get the continuity of care they need. Apply today! Note: this position will receive a Cost of Living Adjustment (COLA) increase of 6.55% which is not reflected in the salary listed in this posting. The effective date of the increase is January 1, 2025 for February 1, 2025 paychecks.

Responsibilities

  • Provide intake case management by coordinating the provision of long-term care services to Oregonians according to rules, regulations and the individual case plans.
  • Contact Oregonians or collateral contacts monthly to set up and monitor service plan and safety needs. This could include in person, the phone, or electronic contact.
  • Work closely and collaboratively with the Oregonian, other agencies, medical and care providers, and community partners to provide benefits, services, and information.
  • Support Oregonians in problem solving issues that impact their health and safety, including culturally specific solutions for culturally specific needs.
  • Determine Long-Term Care Program Eligibility for consumers at the Oregon State Hospital (OSH) and the Oregon Department of Corrections (DOC).
  • Interview Oregonians to determine initial and ongoing eligibility for healthcare, social services, and protective service needs. This can include collateral interviews both in person and on the phone.
  • Develop detailed, individualized, and person-centered case planning for Oregonians.
  • Work in collaboration with the Eligibility team and utilize ONE System to set up, review and update Long Term Care Services, and to review and provide information to Oregonians.

Requirements

  • A valid driver's license and acceptable driving record are required for this position.
  • A bachelor's degree in a Behavioral Science, Social Science, or a closely related field OR a bachelor's degree in any field AND one year of human services related experience OR an associate degree in a Behavioral Science, Social Science or a closely related field AND two years of human services related experience OR three years of human services related experience.

  • Experience conducting assessments to determine eligibility for human service programs.
  • Experience documenting care and/or observations for vulnerable populations.
  • Experience collaborating with other units or agencies to coordinate services for vulnerable populations.
