About the position

Nuvance Health is seeking a Concierge Program Coordinator to assist donors in navigating the healthcare delivery system. This role is essential in nurturing relationships with donors and their families, ensuring strong communication channels between the Foundation and clinical departments. The Coordinator will serve as the primary resource for donors utilizing Nuvance Health and its affiliates for their healthcare needs, providing consistent services based on a Concierge Team call schedule, which includes weeknights, weekends, and holidays. The position is PRN (per diem), allowing for flexible scheduling while ensuring that donors receive the support they need at all times. The Coordinator will be responsible for developing trusting relationships with donors, providing support regarding access to services, referrals, assistance with medical appointments, and addressing billing concerns. Following the receipt of services, the Coordinator will contact donors to gauge their perception of the services provided, promoting donor retention and loyalty. Additionally, the role involves cultivating relationships within various clinical departments to facilitate donor care needs and identify opportunities for improving the care experience for all patients. The Coordinator will track and record HIPAA-compliant information on the Foundation's database, coordinating visits by hospital leadership to further nurture donor relationships. They will also prepare Concierge membership materials, ensuring accurate information capture on donor records. The position requires visiting emergency department patients and families as appropriate to enhance their experience. Compliance with all responsibilities related to the position is essential, along with performing other duties as required.

Responsibilities

  • Serves as a primary source of contact for donors accessing care at Nuvance Health, providing on-call coverage weeknights and weekends as a member of the on-call Concierge program team.
  • Develops trusting relationships and provides support to donors and their families regarding access to services, referrals, assistance with medical appointments, billing concerns, and other matters associated with the donor's care experience at Nuvance Health.
  • Contacts donors following receipt of services to gauge donors' perception of the services and promote donor retention and loyalty.
  • Cultivates relationships within the various clinical departments and care areas, both inpatient and outpatient, to facilitate donor's care needs and to identify new opportunities to improve the care experience for donors and all patients.
  • Tracks and records HIPAA appropriate information on the Foundation's database relative to the visited patients' care experience, notifying and coordinating visits by Hospital(s) leadership to further nurture the donors' relationship with Nuvance Health.
  • Prepares Concierge membership materials, carefully capturing information on donor records and media tab, updating coding on RE & patient database.
  • Visits with ED patients/families, as appropriate, to positively maximize the patient/family experience.
  • Fulfills all compliance responsibilities related to the position.
  • Performs other duties as required.

Requirements

  • Associate degree or equivalent experience.
  • Minimum of 3 years of experience in a related field.
