About the position

The Connections Care Coordinator plays a vital role in coordinating home health care, IV, and DME services for patients requiring care outside of the service areas of Johns Hopkins Howard County Medical Center. This position involves processing patient referrals, ensuring timely service delivery, and collaborating with clinical staff and network providers to maintain high-quality patient care.

Responsibilities

• Coordinate home health care, IV, and DME services for JHHP patients.
• Provide authorizations to network providers and payers.
• Initiate and process initial patient referrals to meet patient needs timely.
• Complete initial review of clinical documentation from referral sources.
• Collaborate with JHHP clinical staff to ensure adequate service provisions.
• Maintain open communication between Connections, vendors, patients, and JHHP.

Requirements

• High school diploma or equivalent required; college preferred.
• Medical Terminology knowledge prior to or within 2 years of hire.
• Experience in a health care related role is preferred.
• Excellent interpersonal and organizational skills.
• Ability to multi-task and communicate effectively in writing and verbally.
• Critical thinking skills and knowledge of clinical documentation in EMR.

Nice-to-haves

• Prior experience in customer service within the healthcare sector.
• Understanding of the Home Care Marketplace and community resources.
