About the position

The Consumer Services Coordinator (CSC) is responsible for coordinating services and supports for individuals with developmental disabilities and their families, ensuring compliance with the Lanterman Developmental Disabilities Services Act. This role involves case management, advocacy, and collaboration with various stakeholders to facilitate the Individual Program Plan (IPP) for consumers in the San Bernardino Metro Adult area.

Responsibilities

  • Provide case coordination services for consumers.
  • Utilize Person-Centered Planning strategies to create and execute the Individual Program Plan (IPP) for consumers.
  • Evaluate consumer progress and implement IPP Addendums as needed.
  • Provide advocacy services and attend relevant meetings, preparing necessary paperwork.
  • Seek and utilize generic resources for consumers and their families.
  • Complete individual or family crisis intervention and document activities appropriately.
  • Complete Special Incident Reports and inform relevant agencies as necessary.
  • Coordinate consumer assessments including medical, psychological, and educational evaluations.
  • Complete placement and liaison activities as required.
  • Ensure Medicaid Waiver standards are maintained and documentation is timely.
  • Arrange placements in residential facilities and training programs as clinically indicated.
  • Complete facility audits as required.
  • Identify issues in vendored programs and make appropriate referrals.
  • Keep the manager informed of caseload status and unusual situations.
  • Participate in case conferences and interagency meetings as needed.
  • Provide case coordination coverage for absent CSCs as directed by the manager.
  • Act as a mentor for new employees as directed by the manager.
  • Complete IRC's orientation and new staff training sessions.
  • Visit assigned residential facilities and day programs.
  • Participate in Quality Assurance evaluations/audits annually.
  • Complete administrative requirements and maintain consumer file documentation.
  • Facilitate the purchase of services identified in the IPP.
  • Prepare for compliance and eligibility reviews.
  • Maintain and expand relevant knowledge through training sessions.
  • Contribute to team harmony and handle change effectively.
  • Organize travel efficiently to visit consumers and families.
  • Utilize bilingual skills as required.
  • Maintain a neat work area and comply with safety rules.
  • Utilize IRC's IT systems and maintain security protocols.
  • Ensure consumers' rights and dignity are upheld in service provision.

Requirements

  • Bachelor's degree from an accredited college or university and one year of experience in case management or related field, or a Master's degree in a relevant field.
  • Bilingual preferred but not required.
  • Ability to establish and maintain effective working relationships.
  • Good verbal and written communication skills.
  • Full use of an automobile and valid California driver's license with liability insurance.
  • Safe driving record.
