The Hospice Continuous Care LPN plays a crucial role in identifying the physical, psychological, social, and spiritual needs of patients and their families. Working under the supervision and direction of the RN Case Manager, the LPN initiates appropriate interventions to support both the patient and their family. This position requires the LPN to follow a comprehensive and responsive plan of care, ensuring that all aspects of the patient's needs are met with compassion and professionalism. In this role, the LPN will provide direct skilled bedside nursing care to patients, ensuring that all care and interventions are documented accurately. The LPN will also be responsible for the appropriate use of medication, durable medical equipment, and supplies, ensuring that the highest standards of care are maintained at all times.
