About the position

The Continuous Care LPN position at Metropolitan Jewish Health System is a vital role within our hospice and palliative care programs, which are designed to provide compassionate support to patients facing life-limiting conditions. This position involves visiting patients in their homes or facilities, delivering direct care in accordance with each patient's individualized plan of care. The LPN will work under the supervision of an RN Case Manager, ensuring that all aspects of patient care are documented accurately using the Netsmart EMR system. This role is essential in monitoring patients' conditions and communicating any changes to the RN Case Manager, thereby facilitating timely interventions and adjustments to care plans. As a Full Time, overnight position, the LPN will be expected to manage a caseload of hospice patients, providing not only medical care but also emotional support to patients and their families during challenging times. The LPN will play a crucial role in easing the burden of managing a life-limiting illness, ensuring that patients receive the highest quality of care in a comfortable and familiar environment. This position requires a compassionate individual who is dedicated to making a difference in the lives of patients and their families during their most vulnerable moments.

Responsibilities

  • Visit hospice patients in their home or facility as directed by the RN Case Manager.
  • Provide direct care to patients following their individualized plan of care.
  • Document each visit accurately using the Netsmart EMR system.
  • Report any changes in the patient's condition to the RN Case Manager.
  • Assist in managing the emotional and physical needs of patients and their families.

Requirements

  • Graduate from an accredited Licensed Practical Nurse Program.
  • Minimum of one year experience as an LPN in Med/surg nursing.
  • At least one year of Hospice or Oncology Nursing experience.
  • Good PC skills are essential.
  • Valid driver's license and reliable transportation for patient visits, except in Manhattan.
