About the position

The Continuum of Care Coordinator (CCC) at Northwest Medical Center is responsible for enhancing patient wellness, minimizing unnecessary readmissions, and ensuring the effective use of in-network healthcare resources. This role involves collaboration with leadership and case management to analyze trends and improve care delivery.

Responsibilities

• Improve patient's level of wellness
• Reduce unnecessary readmissions
• Ensure appropriate utilization of in-network healthcare resources
• Collaborate with Leadership and Case Management to review trends

Requirements

• Bachelor's degree in nursing, social work, or relevant clinical field (preferred)
• Two years of clinical/healthcare experience (preferred)
• Current knowledge of discharge planning, utilization management, case management, and disease management
• Understanding of pre-acute and post-acute venues of care and community resources
• Ability to communicate effectively with patients, families, and healthcare providers
• Ability to aggregate and evaluate patient level data
• Strong problem-solving skills and ability to work in a team-based environment
• Attention to detail and strong drive for self-development

Nice-to-haves

• Supervisor or project leadership experience
• C-AHA BLS Basic Cardiac certification (preferred)
• AZ RN License (preferred)
