About the position

The Virtual Health Coordinator at Wellstar Health System plays a crucial role in promoting excellence in virtual patient care. This position involves the development, implementation, and ongoing management of the Virtual Health program, serving as a clinical role model and educator. The coordinator collaborates with various stakeholders to enhance patient outcomes and ensure high-quality care delivery. Reporting directly to the AVP of Virtual Health, the coordinator is responsible for program goals, operations, and compliance with healthcare standards.

Responsibilities

  • Coordinate virtual health services across sub-specialties from admission to discharge.
  • Collaborate with the Coordinator Teleneuroscience for the stroke program to ensure process alignment.
  • Facilitate collaboration on clinical protocols, order sets, and care paths.
  • Analyze and interpret clinical outcome data to make recommendations.
  • Monitor adherence to clinical protocols and report outcome data to nursing and physician leaders.
  • Ensure compliance with The Joint Commission standards.
  • Meet regularly with sub-specialty program managers for cohesive integration of virtual health.
  • Manage initiatives using project management skills.
  • Serve as a clinical expert for complex patient care situations.
  • Consult with healthcare providers and patients to assess individual care needs.
  • Promote personal and professional growth for self and patient care staff.
  • Identify learning needs and implement educational programs for stroke care.
  • Establish collaborative relationships with physicians and healthcare professionals.
  • Utilize effective communication skills to maintain interpersonal relationships.

Requirements

  • Graduate from an accredited school of nursing.
  • Bachelor's Degree in Science required; Master's Degree preferred.
  • RN License (Single State or Multi-state Compact).
  • Minimum 3 years of acute care clinical experience.
  • Experience in healthcare quality and performance improvement.

Nice-to-haves

  • Experience in project management.
  • Advanced knowledge in assessment, diagnosis, treatment, and evaluation of health problems.
