About the position

The Coordinated Family Support Specialist role at iWork Support Services involves providing essential support to individuals with intellectual disabilities and their families. This part-time position focuses on helping clients develop skills for greater independence and stability while accessing necessary community resources and public assistance programs. The specialist will work closely with families to coordinate services and ensure a supportive environment for individuals living at home.

Responsibilities

  • Assist individuals and families by coordinating services and supports that enable adults to continue living in their family homes.
  • Develop in-home skills to promote independence and stability.
  • Aid in accessing community resources and services.
  • Provide information and resources about the individual's diagnosis and identified supports.
  • Schedule service deliveries, including medical, educational, and vocational appointments.
  • Identify transportation options or services.
  • Assist with future planning to ensure stability and permanency.
  • Maintain accurate documentation of interactions, progress, and other relevant information.
  • Facilitate activities and routines that support consumer stability.

Requirements

  • Bilingual in English and Spanish preferred.
  • Associate's degree or higher from an accredited institution, or a combination of some college coursework and two years of experience in social services, or three years of relevant experience.
  • Experience in developmental, behavioral health, or social services settings is preferred.
  • Strong communication and interpersonal skills.
  • Ability to work independently and demonstrate problem-solving skills.
  • Understanding of mental health and developmental disabilities.
  • Ability to maintain confidentiality and adhere to ethical guidelines.
  • Must have a reliable vehicle, valid driver's license, and car insurance.
  • Must pass a criminal background check.

Nice-to-haves

  • Proficiency in a foreign language (writing, speaking, reading) is highly desired.
