About the position

As a Coordinator at Sunshine State Health Plan, you will play a crucial role in transforming the health of our communities by supporting community connection activities. This position is designed to connect our 28 million members to essential community resources that will aid in their care management journey. You will be responsible for providing members with information about available community resources and assisting the care team in identifying support options for members. Your efforts will ensure that members receive the necessary health education and resources in a cost-effective manner. In this role, you will support the coordination of community outreach resources available to members, promoting awareness of the care and services offered. You will serve as a point of contact for members regarding inquiries about community and care resources, ensuring they are well-informed about the opportunities available to them. Additionally, you will assist in the preparation of member-related correspondence and educational materials, facilitating successful community connections. Your responsibilities will also include documenting and maintaining all community resources to ensure compliance with health plan standards and policies. You will provide assistance to the clinical team, which includes nurses and social workers, by conducting outreach, community education, and offering informal guidance and support to members. Non-clinical general health assessments will be part of your duties, allowing you to refer members to appropriate care and services, resolve concerns, and gather necessary information for medical providers. You will conduct assessments related to home safety, community resources, transportation, and employment, among others, to ensure members receive the appropriate referrals. Outreach efforts may involve telephonic and in-person communication to locate individuals and families in the community who may be difficult to reach. Home visits and visits to community organizations may also be required. A working knowledge of Social Determinants of Health (SDOH) barriers will be beneficial in this role. You will be expected to perform other duties as assigned and comply with all policies and standards.

Responsibilities

  • Support community connection activities by connecting members to community resources.
  • Provide members with information about known community resources and assist the care team in identifying support options.

  • Serve as a support for members on community and care resource inquiries and opportunities.

  • Conduct non-clinical general health assessments to refer members to appropriate care/services.
  • Conduct non-medical assessments such as home safety and community resource assessments.
  • Perform telephonic and/or in-person outreach to locate hard-to-reach individuals and families.
  • Make visits to individual homes and/or community organizations as needed.
  • Maintain a working knowledge of Social Determinants of Health (SDOH) barriers.
  • Perform other duties as assigned and comply with all policies and standards.

Requirements

  • High School diploma or GED required.
  • 1 - 2 years of related experience required.
