About the position

As a Coordinator at Sunshine State Health Plan, you will play a crucial role in transforming the health of our communities by connecting our 28 million members to essential community resources. This position is field-based, requiring up to 75% travel to visit members in their homes across various counties in Florida, including Lee, Sarasota, Charlotte, Desoto, Collier, Manatee, Hardee, Highlands, Glades, and Hendry. Candidates must reside in one of these counties and have the ability to work remotely. Your primary responsibility will be to support community connection activities, ensuring that members receive the necessary care resources in a cost-effective manner. You will provide members with information about known community resources and assist the care team in identifying additional support for members, while also offering health education as appropriate. In this role, you will document and maintain all community resources to ensure compliance with health plan standards and policies. You will also assist the clinical team, which includes nurses and social workers, by conducting outreach, community education, and providing informal guidance and support to members. Your duties will include conducting non-clinical general health assessments to refer members to appropriate care and services, resolving concerns on their behalf, and gathering information for medical providers. Additionally, you will perform non-medical assessments related to home safety, community resources, and transportation, among others, to facilitate appropriate referrals. You will engage in telephonic and in-person outreach to locate individuals and families in the community who may be hard to reach, and may also visit individual homes and community organizations. A working knowledge of Social Determinants of Health (SDOH) barriers is essential for this position, as it will help you better understand and address the challenges faced by our members. You will also be expected to comply with all policies and standards while performing other duties as assigned.

Requirements

  • High School diploma or GED required.
  • 1 - 2 years of related experience required.
  • Field-based work experience highly preferred.
  • Community Health Worker experience and certification preferred.
