Under the general supervision of the Enhanced Care Management Department Leadership, the Coordinator - Enhanced Care Management is responsible for working effectively with the integrated Enhanced Care Management team to facilitate and support Enhanced Care Management team activities. The coordinator is responsible for skillfully engaging IEHP Members, in order to enhance Member activation in their own health and wellness practices, as well as supporting the Enhanced Care Management team with administrative duties. Commitment to Quality: The IEHP Team is committed to incorporate IEHP's Quality Program goals including, but not limited to, HEDIS, CAHPS, and NCQA Accreditation.

Responsibilities

  • Work effectively with the integrated Enhanced Care Management team.
  • Facilitate and support Enhanced Care Management team activities.
  • Engage IEHP Members to enhance their activation in health and wellness practices.
  • Support the Enhanced Care Management team with administrative duties.

Requirements

  • Two (2) years customer service experience in any setting.
  • Experience of successfully working within a team.
  • Experience in data entry.
  • High school diploma or GED required.
  • Associate's degree or Bachelor's degree from an accredited institution preferred.
  • Medical Assistant Certification or Certified Nursing Assistant preferred.

Nice-to-haves

  • Managed care setting experience preferred.
  • Knowledge of Utilization Management including referral process, ICD-10 and CPT codes preferred.
  • Knowledgeable about evidenced based communication such as Motivational Interviewing, or similar empathy-based communication strategies.
  • Understanding of and sensitivity to multi-cultural community.
  • Understanding of and sensitivity to mental health conditions and addictive disorders.
  • Awareness of the impact of unmitigated bias and judgement on health; commitment to addressing both.
  • Understanding of, and a commitment to, high performing team practices.
