About the position

The Patient Safety Coordinator at Kaiser Permanente is responsible for promoting a culture of safety within the healthcare environment. This role involves implementing patient safety standards and guidelines, ensuring compliance with various regulatory bodies, and developing proactive processes to identify and mitigate risks. The coordinator will work closely with hospital staff and leadership to enhance patient safety and quality of care, while also providing education and support related to infection prevention and quality improvement.

Responsibilities

  • Collaborates with various departments/services in the implementation of programs related to National Patient Safety Goals, Leap Frog, and NQF.
  • Monitors compliance on patient safety indicators through direct observation and audit activities.
  • Promotes MHS patient safety culture by rounding on inpatient care units and ambulatory areas, coaching staff and physicians on patient safety issues.
  • Develops and implements infection prevention and control programs, identifying and escalating infection control problems.
  • Keeps abreast of current federal and state regulations, as well as professional standards and industry best practices.
  • Assists in reviewing and developing Plans of Correction for deficiencies noted during inspections, providing written copies to the Administrator.
  • Provides patient safety education to staff, members, and physicians.
  • Develops and formulates patient safety reports to be shared with staff and leadership as needed.
  • Consults with department managers to assist in eliminating/correcting problem areas and improving service.
  • Conducts patient safety risk assessments and designs effective patient safety plans, completing annual evaluations.
  • Facilitates and coordinates implementation of patient safety system changes and special projects.
  • Communicates and disseminates information on patient safety best practices at all levels of staff and leadership.
  • Represents the medical center on patient safety matters in various programs as assigned.
  • Participates in community and healthcare industry activities to enhance patient safety and clinical risk management.
  • Collects and analyzes data related to patient safety programs and initiatives.
  • Performs risk assessments and utilizes quality improvement tools to facilitate changes in processes for patient safety.
  • Reports medical errors and near misses, overseeing the management of medical error information.
  • Participates in the development and implementation of internal audits for compliance with various regulatory bodies.
  • Coordinates the audit process, analyzing and reporting findings to leadership and quality committees.
  • Plans and facilitates Quality meetings for Kula Hospital and Lanai Hospital.

Requirements

  • Minimum of two (2) years of nursing experience.
  • Bachelor's degree in nursing, health services administration, business or public administration, or public health OR four (4) years of experience in a directly related field.
  • Infection Control Certificate within 24 months of hire.
  • Registered Nurse License (Hawaii) required at hire.
  • Basic Life Support certification.

Nice-to-haves

  • Minimum one (1) year experience in clinical risk management, patient safety, and performance/quality improvement.
  • Certification as a Certified Professional in Healthcare Risk Management (CPHRM).
  • Training and certification in quality improvement and safe culture programs.
  • CPHQ certification.
  • Master's degree in Business Administration or Healthcare preferred.
