About the position

The Coordinator Placement Partnership Program (PPP) is a full-time role focused on supporting foster care youth and their families. This position is essential for preserving foster care placements and facilitating the re-entry of youth from psychiatric hospitals, residential treatment centers, and detention centers. The coordinator provides mobile, in-home therapeutic crisis intervention services, develops individualized care plans, and collaborates with Dependency Case Managers and Child Protective Investigators to monitor progress and assess needs for further intervention.

Responsibilities

  • Provide mobile, in-home therapeutic crisis intervention services to children and families.
  • Create individualized plans of care for each client.
  • Coordinate supports needed to sustain identified placements.
  • Work in cooperation with Dependency Case Managers and/or Child Protective Investigators to monitor progress of children and families receiving services.
  • Assess the need for further intervention based on client progress.

Requirements

  • Master's degree in a human service-related field or a bachelor's degree with significant experience in dependency and crisis support.
  • Minimum of one (1) year experience working with at-risk children and families with child welfare involvement.
  • Knowledge in crisis intervention, family dynamics, childhood/adolescent development, and community resource networks.
  • Excellent verbal and written communication skills.
  • Strong organization and time management skills.
  • Valid Florida driver's license and auto liability insurance.

Nice-to-haves

  • Bilingual in Spanish/Creole preferred.
