About the position
The County Caseworker 2 (Supports Coordinator) position at Northumberland County is focused on improving the quality of life for residents with intellectual disabilities and developmental services. The role involves empowering individuals to achieve independence, advocating for their needs, and coordinating services to ensure they receive appropriate care and support. This position is integral to the community, working closely with families and service providers to enhance the lives of vulnerable citizens.
Responsibilities
- Obtain social information for consumers for the development of treatment plans and enrollment into services under supervision.
- Assist consumers in filing applications for benefits.
- Participate in the development of Individual Service Plans for each consumer.
- Monitor Residential and Day Program providers to ensure implementation of Individual Program Plan services.
- Maintain files, records, and documentation necessary for consumer care and life management plans.
- Act as an advocate for consumers to ensure appropriate treatment is approved.
- Coordinate and direct meetings to assess consumers' progress.
- Access supportive services for clients in the community, including budgeting and social skills.
- Attend resident reviews at various service providers to ensure adequate service levels.
- Provide crisis intervention and emergency services as coordinated by the supervisor.
- Provide technical assistance to service providers in maintaining compliance with regulations.
Requirements
- Six months of experience as a County Caseworker 1;
- Successful completion of the County Social Casework Intern program;
- A bachelor's degree with a social welfare major;
- A bachelor's degree supplemented by 12 college credits in related social sciences and one year of professional social casework experience;
- Any equivalent combination of experience and training including 12 college credits in related social sciences.
