About the position

The CPST Substance Use Specialist plays a crucial role in supporting adults facing addiction and mental health challenges. This position focuses on enhancing clients' functioning within their communities through coaching, skill development, and care coordination. The specialist works closely with clients to improve their coping strategies, relationships, and daily living skills while ensuring they have access to necessary community resources.

Responsibilities

  • Assists SUD counselors in conducting screenings and assessments of clients through the person-centered treatment planning process.
  • Assists clients and family members in identifying strengths, needs, resources, and developing effective strategies.
  • Helps clients individually and in IOP groups to identify behaviors associated with their substance disorder that interfere with daily living.
  • Provides supportive intervention to help clients identify and apply coping strategies.
  • Participates in team observation support and supervision of clients experiencing withdrawal symptoms.
  • Provides individual and group interventions as requested by the SUD Manager.
  • Coordinates community resources to address financial, housing, food benefits, employment, and transportation needs.
  • Communicates with internal and external providers to foster coordination of care.
  • Assists nurses in supporting clients to manage chronic medical conditions and establish good health routines.
  • Maintains knowledge of available providers and resources in the community for successful discharge planning.
  • Participates in supervision, team conferencing, and agency meetings.
  • Maintains detailed, accurate, and timely documentation in health records.
  • Enhances and maintains professional expertise through educational activities and trainings.
  • Meets requirements for client contact time according to treatment plan and agency productivity targets.
  • Follows agency policies and standards for client rights and confidentiality.

Requirements

  • Experience in substance use disorder treatment or mental health support.
  • Strong communication and interpersonal skills.
  • Ability to work collaboratively with clients, families, and other providers.
  • Knowledge of community resources and support systems for addiction and mental health.
  • Experience in documentation and maintaining health records.

Nice-to-haves

  • Certification in substance use counseling or related field.
  • Experience in group facilitation or therapy.
  • Familiarity with person-centered treatment planning.
