About the position

The CPST (Community Psychiatric Support Team) Substance Use Specialist plays a crucial role in supporting adults facing addiction and mental health challenges. This position focuses on enhancing clients' functioning within their communities through coaching, relationship improvement, symptom management, and care coordination with other providers. The specialist connects clients with community resources to facilitate successful discharge planning and overall well-being.

Responsibilities

  • Assist SUD counselors in conducting screenings and assessments of clients through the person-centered treatment planning process.
  • Help clients and family members identify strengths, needs, resources, and develop effective strategies.
  • Support clients individually and in IOP groups to identify behaviors associated with their substance disorder that interfere with daily living and community integration.
  • Provide supportive intervention to help clients identify and apply coping strategies.
  • Participate in team observation support and supervision of clients experiencing withdrawal symptoms.
  • Provide individual and group interventions as requested by the SUD Manager.
  • Coordinate community resources to address financial, housing, food benefits, employment, and transportation needs.
  • Communicate with internal and external providers to foster coordination of care.
  • Assist nurses in supporting clients to manage chronic medical conditions and establish good health routines.
  • Maintain knowledge of available providers and resources in the community for successful discharge planning.
  • Participate in supervision, team conferencing, department, and agency meetings.
  • Maintain detailed, accurate, and timely documentation in health records.
  • Enhance and maintain professional expertise through educational activities and trainings.
  • Meet requirements for client contact time according to treatment plan and agency productivity targets.
  • Follow agency policies and standards for client rights and confidentiality.

Requirements

  • Associate's degree in a human service field such as psychology, counseling, education, or rehabilitation.
  • At least 2 years of experience working in a behavioral health or human service setting.
  • Knowledge of human development and family systems issues.
  • Understanding of mental health and substance use diagnoses.
  • Knowledge of community resources for clients and families with behavioral health disorders.
  • Advocacy and coaching skills.
  • Valid driver's license and automobile liability insurance coverage.
