About the position

The Crisis Case Manager is responsible for providing direct crisis intervention services to the public, focusing on preventing disabilities and promoting mental and physical health. This role includes on-call crisis coverage, conducting assessments, and coordinating services for individuals with various mental health and developmental challenges. The position requires a high degree of independence and collaboration with community providers and law enforcement.

Responsibilities

• Provides direct face-to-face and telephone crisis intervention services.
• Conducts risk assessments and provides crisis counseling.
• Supports behavioral de-escalation through skills coaching.
• Arranges admission to inpatient psychiatric facilities for individuals under Chapter 51 detentions.
• Works a rotating on-call schedule, responding to emergencies within a 30-minute timeframe.
• Develops crisis plans for continuity of care after initial responses.
• Acts as a liaison between consumers and community agencies for follow-up services.
• Evaluates the need for protective services and alternative placements.
• Monitors and manages cases for individuals with Mental Health Commitments or Settlement Agreements.
• Generates and maintains clinical records and reports as per state law and agency requirements.

Requirements

• Bachelor's Degree in Social Work, Psychology, Rehabilitation, Special Education or related field.
• One to two years of related experience or training, or equivalent combination of education and experience.
• Valid Wisconsin driver's license and adequate insurance as required by County Policy.
• Ability to respond to emergencies within a 30-minute timeframe.
