About the position

The Community Transitions Critical Time Intervention (CTI) Coach at West Central Behavioral Health is responsible for providing community-based, time-limited interventions to individuals facing behavioral health challenges. The role focuses on supporting successful transitions and community integration, utilizing an intensive case management model to link clients with necessary clinical and community supports. The CTI Coach aims to decrease psychiatric hospitalizations and emergency department visits by promoting recovery and facilitating access to services over a nine-month period.

Responsibilities

  • Provide CTI interventions for an assigned group of clients, coordinating and monitoring activities of a documented transition plan.
  • Utilize a person-centered approach to develop, implement, evaluate, and revise goals and plans in partnership with participants.
  • Provide individual skill building to promote independent living and system management.
  • Ensure immediate changes are made in transition plans as clients' needs change.
  • Consult with community agencies and natural supports to ensure coordination in the service process.
  • Conduct brief, evidence-based, and developmentally appropriate screenings and arrange for more detailed assessments when indicated.

Requirements

  • A Bachelor's degree in a related human service field and at least 2 years of experience working with behavioral health, developmentally delayed, incarcerated, and/or substance use populations; or a high school diploma or GED with at least 4 years of relevant experience.
  • A Certified Recovery Support Worker (CRSW) Certification through a program endorsed by the State of New Hampshire is a plus.
  • Demonstrated knowledge of social and human service providers, public benefit systems, and referral processes.
  • Experience providing case management and individual assessments to clients.

Nice-to-haves

  • Additional certifications or credentials related to behavioral health or case management.
  • Documented completion of training in Critical Time Intervention (CTI), Stages of Change Training, Motivational Interviewing, Ethics, Cultural Competence, Person Centered Planning, and Suicidal Prevention.
  • Experience working with diverse populations and understanding of cultural competence.
