Responsibilities
- Build and maintain strong relationships with clients and their families.
- Answer inquiries and address concerns from clients.
- Coordinate care schedules to ensure client satisfaction.
- Collaborate with the care team for seamless service delivery.
- Proactively solve problems and ensure effective communication.
Requirements
- At least 3 years of Support Coordination Experience.
- 2 work references available for contact.
- Strong problem-solving and organizational skills.
- Excellent verbal and written communication abilities.
- Proficiency in using Google Apps (Gmail, Drive, Voice, Meet, Sheet, Docs, and Hangouts).
- Access to a computer or laptop with internet connectivity.
- Willingness to travel for work up to 50% of the time.
- Own a reliable vehicle for fieldwork.
- Hold a valid Driver's License.
