As a Customer Success Liaison, you will work on the front lines with physicians and clinical teams, delivering Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) services. Your role is crucial in ensuring that patients have access to our innovative care solutions, helping them live healthier, longer lives. In this position, you will build relationships with clinicians and their office staff to support patient enrollments in Nsight's RPM and CCM services. You will facilitate high-value, patient-centered care by working directly in clinics to enroll patients, organize data, and enhance communication between providers and patients. Additionally, you will serve as the bridge between patients and clinical teams, providing education about Nsight's care programs and driving engagement. You will also implement team-based, patient-centered processes that improve chronic disease management and collaborate with care teams to ensure timely onboarding and care plan adherence.

Responsibilities

  • Partner with clinicians and their office staff to support patient enrollments in Nsight's RPM and CCM services.
  • Facilitate high-value, patient-centered care by working directly in clinics to enroll patients, organize data, and enhance communication between providers and patients.
  • Provide education about Nsight's care programs and drive engagement between patients and clinical teams.
  • Implement team-based, patient-centered processes that improve chronic disease management.
  • Collaborate with care teams to ensure timely onboarding and care plan adherence.

Requirements

  • 5-10 years in customer success, account management, or client services, preferably in healthcare.
  • A track record of driving patient enrollments and success in a fast-paced, client-facing role.
  • Strong communication and relationship-building skills, with the ability to educate and inspire.
  • Experience with HubSpot, EHR/EMR systems, and customer service tools.
