About the position

The Caseworker / Investigator position with Liberty Healthcare and the Pennsylvania Department of Human Services focuses on ensuring the wellbeing of adults with disabilities by investigating reports of abuse, neglect, exploitation, or abandonment. The role involves conducting investigations, documenting findings, and managing active cases, all while providing support and developing plans to mitigate risks.

Responsibilities

• Investigate reports of abuse, neglect, exploitation, or abandonment of adults living with disabilities.
• Conduct investigations in residences throughout Dauphin County and surrounding areas.
• Document findings and determine if reports are substantiated.
• Develop plans to mitigate or eliminate risks identified during investigations.
• Manage active cases and provide ongoing support to clients.

Requirements

• Bachelor's degree preferred, or relevant experience in behavioral health, mental health, or intellectual and developmental disabilities services.
• Experience in field-based healthcare or human services.
• Background in service coordination, supports coordination, or program specialist roles.
• Experience in child, family, and youth services case management.
• Law enforcement experience or case management in protective services.

Nice-to-haves

• Knowledge of Social Assistance Management Systems (SAMS).
• Familiarity with Home and Community Service Information Systems (HCSIS).
• Spanish speaking skills.
