About the position
The Day Center Aide (CENA) at LifeCircle PACE is responsible for supporting participants with activities of daily living (ADLs) and instrumental activities of daily living (IADLs) as per their individualized care plans. This full-time role involves providing care in both the center and participants' homes, assisting with meal services, transportation, and medication deliveries, while ensuring thorough documentation and adherence to safety protocols. The position is integral to delivering patient-centered care to frail seniors who wish to remain in their homes.
Responsibilities
- Provide ADL and IADL care supports for each participant in the center or home as directed.
- Assist participants with active engagement in scheduled programming at the center.
- Provide assistance with meal service at the center.
- Support participants in getting to and from the center by providing bus assistance.
- Accompany participants to scheduled appointments as directed by the supervisor.
- Assist with medication deliveries as assigned.
- Complete documentation thoroughly and timely.
- Perform safety, security, and cleaning activities as scheduled per organizational protocols.
- Follow established departmental policies and procedures.
- Provide home care supports in the community as needed and assigned by the supervisor.
Requirements
- Minimum of 3 years of experience as a CNA in a geriatric setting.
- 1-year experience working with the frail or elderly population.
- A current, unencumbered, active CENA certification in the State of Michigan is required.
- Must have reliable transportation and a current valid Michigan driver's license.
- Possess current CPR certification by AHA or willing and able to obtain as offered by LifeCircles.
