About the position

The Day Program Coordinator Trainee at Highland Rivers Health is responsible for managing the daily operations of the Day Program for individuals with intellectual and developmental disabilities. This role focuses on providing activities, services, and learning opportunities to help these individuals live independently within their communities. The coordinator will oversee various aspects of the program, ensuring quality-driven social, recreational, vocational, and educational support for participants and their families.

Responsibilities

  • Daily contact with individuals
  • Monthly documentation (summaries, HRST updates, DDP ISP review notes)
  • Oversees locked medications for individuals and returns unused medications, maintaining monthly MARs
  • Individual Service Plan reviews, development, and implementation
  • Trainings for staff as indicated in individuals' service plans/HRST/Safety plans and protocols
  • Liaison with community, support coordination, administration, families/caregivers, contractors, regional office, intake and evaluation, transportation, DFCS
  • Intimate knowledge of DD Services and funding sources (NOW/COMP Waivers, state funded) and service rates
  • Budgeting and maximizing service units of individuals in all services
  • Management, maintenance, and audits of electronic health records, including ECR page
  • Document review (PA, DMA6 & SMA7, ISP, HRST, SIS)
  • Completes discharges and paperwork
  • Crisis intervention, monitor and maintain health and safety of individuals
  • Completes incident reports (non-critical, critical, death reports and follow-ups)
  • Assist with instructor duties, as needed (CAI, feeding, toileting, supervision, etc.)
  • Transporting individuals into the community, to/from home and during emergencies
  • Scheduling staff
  • Quarterly DDP Documentation
  • Required Training
  • Monthly Staff Meetings
  • Other duties as assigned.
