About The Position

The DAC Peer Support Specialist is responsible for providing education and support to individuals with Serious Mental Illness (SMI) and/or Severe and Persistent Mental Illness (SPMI) who are transitioning from inpatient settings. This role involves working collaboratively with various stakeholders to ensure a smooth transition and support for members in their home communities. The position is hybrid, requiring the candidate to reside in North Carolina and travel as needed for meetings and member support.

Requirements

  • High School Diploma or equivalent.
  • NC Certified Peer Support Specialist Program Certification (NC CPSS).
  • At least 1 year of experience working with individuals with SMI/SPMI and their families.
  • Lived experience with mental health conditions, substance abuse, or trauma is required.
  • Proficiency in Microsoft Excel, Word, and Outlook.

Nice To Haves

  • Experience providing peer support to individuals in and out of the Justice System.

Responsibilities

  • Provide Care Team Support to members transitioning from inpatient settings.
  • Support the development and delivery of a whole person approach to Care Management.
  • Collaborate with Alliance staff, behavioral health providers, and community partners to support members.
  • Assist DAC Care Coordinator with Core Re-Entry Transitional Care Coordinator Functions.
  • Meet with clients regularly to offer one-on-one support and counseling.
  • Develop recovery plans, coping methods, and rehabilitation strategies with clients.
  • Arrange medical appointments, therapy sessions, and support groups for clients.
  • Report any unusual behavior or recovery deviations to relevant professionals.
  • Liaise with family members and medical professionals regularly.
  • Coordinate logistics for clients' medical appointments and therapy sessions.
