About the position

Mercy Primary Care is seeking a full-time Diabetes Educator to join our team in Oklahoma City, Oklahoma. The Diabetes Educator plays a crucial role in providing comprehensive education and support to patients diagnosed with diabetes within the outpatient setting. This position involves collaborating with healthcare teams to develop and implement personalized diabetes management plans, empowering patients to achieve optimal health outcomes and improve their quality of life. The educator will work closely with patients to assess their individual needs, provide tailored education on diabetes management, and support them in making lifestyle changes that promote better health. This role is essential in helping patients understand their condition, manage their symptoms, and navigate the complexities of diabetes care. The Diabetes Educator will also be responsible for staying updated on the latest diabetes research and treatment options to provide the best possible care to patients. Additionally, the position may involve conducting group education sessions and workshops to reach a broader audience and enhance community awareness about diabetes management.

Responsibilities

  • Provide comprehensive education and support to patients diagnosed with diabetes.
  • Collaborate with healthcare teams to develop personalized diabetes management plans.

  • Conduct individual assessments to determine patient needs related to diabetes management.
  • Deliver tailored education on diabetes management, including lifestyle changes and medication adherence.

Requirements

  • Bachelor's degree in Dietetics, Health Education, or related field.
  • Certified Diabetes Care and Education Specialist (CDCES) credential required.
  • Clinical experience in diabetes management and education, preferably in a hospital or outpatient setting.
  • At least 2 years of inpatient or outpatient RD experience strongly desired.
