About the position

The Diabetic Educator position at Nebraska Methodist Health System focuses on providing individualized diabetes education to patients, ensuring they receive comprehensive nutrition care plans tailored to their specific needs. The role involves collaborating with healthcare professionals, assisting patients in setting personal health goals, and addressing barriers to successful diabetes management. This full-time remote position emphasizes patient interaction, education, and support, contributing to the overall mission of delivering exceptional care to the community.

Responsibilities

  • Provides individualized diabetes education based on identified patient needs.
  • Creates comprehensive nutrition care plans for patients by identifying pertinent data from medical/diet/social histories.
  • Integrates pathophysiology, calorie, nutrient and fluid intake vs. patient needs into nutrition recommendations.
  • Provides nutrition education to patients and families through counseling, education, and training materials.
  • Assists patients to identify and set personal goals for improvements in nutrition and exercise.
  • Identifies and addresses barriers that prevent patient success, such as financial or knowledge deficits.
  • Charts diabetes education accurately in the Electronic Medical Records (EMR).
  • Follows up with high-risk patients to provide continuous support and ensure they are scheduled for follow-up appointments.
  • Educates other clinic staff to facilitate ongoing learning and development.
  • Collaborates with physicians and health coaches to provide quality care.

Requirements

  • Bachelor's Degree required, preferably in Nutrition and Dietetics.
  • 3 years of previous clinical experience preferred.
  • Certified Diabetes Care and Education Specialist (CDCES) preferred.
  • Registered Dietician (RD), Registered Dietician Nutritionist (RDN) or Registered Nurse (RN) required.
  • Knowledge of patient education and critical thinking skills for problem-solving.
  • Strong verbal communication skills for positive interaction with patients and healthcare team members.

Nice-to-haves

  • Experience in patient interaction.
  • Familiarity with electronic medical records (EMR).
