About the position

The Diabetic Educator role at Wake Forest Baptist Health focuses on providing comprehensive diabetes self-management education (DSME) to patients and their families. The position requires mastery in diabetes knowledge and treatment therapies, utilizing behavioral and case management interventions to enhance patient care. The educator will work collaboratively with a multidisciplinary team to deliver age-appropriate education and ensure adherence to the American Diabetes Association (ADA) standards of care.

Responsibilities

  • Provide quality diabetes self-management education (DSME) to referred patients and their families.
  • Utilize behavioral and case management interventions in patient care.
  • Collaborate with a multifaceted education instructional team to deliver comprehensive diabetes education.
  • Assess, plan, and document patient management effectively.
  • Manage intensive therapies requiring critical thinking and independent judgment.
  • Evaluate patient progress and revise care plans as necessary.
  • Maintain extensive follow-up for complex patient populations.

Requirements

  • Graduation from an accredited School of Nursing with two years of diabetes-related work experience.
  • Current license or certification to practice as a Registered Nurse (RN) in North Carolina, Registered Dietitian, Registered Pharmacist, Social Worker, Family Counselor, Health Educator, or Exercise Physiologist.
  • Strong oral and written communication skills.
  • Organizational and effective time management skills.

Nice-to-haves

  • Bachelor's degree in Human Nutrition, Foods and Nutrition, Biological Sciences, Dietetics, Sociology, Public Health, Psychology, or Pharmacy.
  • Certified Diabetes Educator (C.D.E.) certification preferred, or must obtain within two years.
  • Patient-oriented educational experience with a broad knowledge of diabetes and its interrelation to co-existing health problems.
