About the position

The position involves managing patient care in home therapies programs, ensuring cost-effective clinical operations while providing guidance to an interdisciplinary team. The role focuses on growing assigned Home Therapies programs and maintaining high standards of patient care for individuals with End-Stage Renal Disease (ESRD).

Responsibilities

  • Manage the operations of multiple Home Therapies programs, including growth, patient retention, program costs, operational methods, scheduling, and staffing.
  • Direct activities related to strategic plans, goals, and objectives, ensuring alignment with regional and company objectives.
  • Implement quality goals and action plans to achieve quality standards.
  • Collaborate with operations leaders and quality teams to promote home therapies growth.
  • Manage profit and loss for the assigned program, ensuring optimal performance and budget adherence.
  • Oversee all aspects of patient care from admission through discharge for Home Therapies patients.
  • Provide direction and support to staff for safe and effective home therapy delivery.
  • Assess patient and caregiver readiness for home dialysis treatments and train them on equipment operation.
  • Support patient satisfaction initiatives and embody company values in patient experience efforts.
  • Ensure staff competency in operating home dialysis equipment and maintaining certifications.
  • Manage program supplies and equipment tracking, ensuring timely replenishment.
  • Direct data entry activities related to patient treatment and regulatory compliance.
  • Orient and support new home therapies staff, ensuring compliance with risk management initiatives.
  • Responsible for hiring, coaching, and performance management of employees.

Requirements

  • Graduate of an accredited School of Nursing (R.N.)
  • Current appropriate state licensure
  • Minimum of one year nursing experience
  • Minimum of six months dialysis nursing experience
  • 3+ years' supervisory or project/program management experience preferred
  • Demonstrated management skills necessary for supervising dialysis personnel
  • Excellent communication and customer service skills
  • Continuous quality improvement and relationship development skills
  • Results orientation and team building abilities
  • Motivating employees and performance management skills
  • Decision-making capabilities
  • Successfully complete CPR Certification with maintenance as required.
