About the position

The Dietitian at Cassena Care is responsible for assessing the nutritional needs of residents in a nursing and rehabilitation center. This role involves conducting nutritional assessments, developing individualized nutrition care plans, and collaborating with healthcare teams to ensure optimal dietary support for residents. The position is part-time and per diem, focusing on evening shifts.

Responsibilities

  • Determine the nutritional status of residents through assessments based on facility policies.
  • Complete all RAI (Resident Assessment Instrument) requirements as per facility policies.
  • Assess the adequacy of calorie, macronutrient, and fluid intake within facility guidelines.
  • Collect and review anthropometric data and biochemical parameters to determine nutritional needs.
  • Collaborate with residents, families, and healthcare teams to assess nutrition education needs.
  • Review and assess monthly and/or weekly weight data.
  • Consider religious, cultural, and ethnic factors in nutrition assessments.
  • Develop individualized nutritional interventions and document them in care plans.
  • Identify appropriate tube feeding formulas to meet resident requirements.
  • Monitor and evaluate the nutrition care and outcomes of residents.
  • Participate in resident care team meetings and discussions.
  • Disseminate diet orders and food preferences to diet aides accurately and timely.
  • Maintain accurate computer records related to residents' dietary needs and preferences.
  • Monitor residents during mealtime to ensure proper meal service.

Requirements

  • Bachelor's degree in a related field (RD preferred but not required).
  • Experience with MDS, PCC, and Prime preferred but willing to train.

Nice-to-haves

  • Experience in geriatrics or long-term care settings.
