About the position

The Full-time Dietitian at Premise Health will collaborate with a multidisciplinary team to provide comprehensive nutritional care to members aged 2 and up. This role involves conducting nutritional assessments, developing individualized care plans, and presenting nutrition wellness programs. The Dietitian will also document care plans and monitor health outcomes, ensuring members receive the necessary education and resources to improve their health.

Responsibilities

• Collaborates with a multidisciplinary team to deliver optimum member care.
• Conducts comprehensive nutritional assessments on members ages 2 and up.
• Identifies health risks and provides evidence-based recommendations and education.
• Develops individualized plans of care and assists members in creating SMART goals.
• Documents nutrition care plans and member progress in medical records in a timely manner.
• Reviews HEDIS standards for members with Diabetes or HTN and monitors outcomes.
• Plans and presents nutrition wellness programs based on client needs.
• Evaluates nutrition offerings with cafeteria or food service vendors.

Requirements

• Bachelor's degree with coursework approved by the Academy of Nutrition and Dietetics.
• Documentation of state license/registration if applicable.
• Copy of Commission on Dietetic Registration Certificate Card and CDR number.
• Three years of experience as a registered dietitian in a clinical setting.
• Experience in public speaking and presentations.

Nice-to-haves

• Master's degree preferred.
• Additional certifications in wellness/fitness specialties preferred.
• Experience in health promotion or work site wellness preferred.
