About the position

The Director of Case Management at Roxborough Memorial Hospital is responsible for leading and supervising the case management team, which includes case managers, social workers, and discharge planners. This role involves developing staff and systems to operate a comprehensive Case Management Program that aligns with the hospital's mission and meets patient and family needs. The Director will utilize continuous quality improvement tools to integrate and coordinate services effectively.

Responsibilities

• Develop and implement a comprehensive Case Management Program.
• Provide leadership and supervision to case managers, social workers, and case management coordinators/discharge planners.
• Assess patient needs and design appropriate services in line with hospital mission.
• Integrate and coordinate services using continuous quality improvement tools.

Requirements

• Minimum 5 years post-graduate experience from an accredited school of nursing with a current state Registered Nurse license.
• Minimum 5 years post-graduate experience from an accredited school of Social Work for Licensed Clinical Social Worker and/or Doctorate in a related field.
• At least 5 years of experience in a Case Management position.
• Analytical ability for problem identification and assessment of data/statistics.
• Certification in Case Management, BS or BSN or related field preferred.
• Current BCLS (AHA) certificate upon hire and maintain current certification.
• Knowledge of Milliman Criteria and InterQual Criteria preferred.
• Basic to intermediate computer skills.
• Current Behavioral Violence Prevention Training within 3 months of hire and maintain current.
