About the position

The Value Based Care Clinical Program Director is responsible for driving the culture of Value-Based Care within the organization and advancing strategic goals through clinical expertise. This role involves developing, implementing, and managing programs that enhance the organization's performance in value-based care. The director will analyze complex data trends and outcome metrics to identify performance improvement opportunities and collaborate with clinical leadership to achieve these improvements. The position requires effective guidance to staff at all levels and serves as a critical problem solver and subject-matter expert.

Responsibilities

  • Drive the culture of Value-Based Care within the organization.
  • Develop, implement, and manage programs to enhance value-based care performance.
  • Analyze complex data trends and outcome metrics to identify performance improvement opportunities.
  • Collaborate with clinical leadership to realize performance improvements.
  • Provide guidance to staff at all levels, both within and outside the value-based care department.
  • Function as a critical problem solver and subject-matter expert.

Requirements

  • Minimum 5 years healthcare experience.
  • Clinical license required (e.g. Nurse Practitioner).
  • Demonstrated overall knowledge of the healthcare business environment, including Medical Groups, IPAs, and Health Plans.
  • Experience working in a fast-paced environment with minimal supervision and managing multiple priorities.
  • Proven excellent leadership and relationship management skills.
  • Ability to manage and motivate direct and indirect reports.
  • Experience in a team-oriented, collaborative environment.
  • Experience in leading projects and providing expert consultative guidance on change initiatives.
  • Demonstrated analytical, creative thinking, and problem-solving skills.
