About the position

The Direct Care Worker/Instructor at REACH Unlimited Inc. is responsible for providing active treatment and support to individuals with intellectual and developmental disabilities. This role involves implementing training programs, assisting with daily living activities, and ensuring the safety and well-being of assigned clients. The position requires effective communication, documentation of services, and adherence to company policies and procedures.

Responsibilities

  • Provide supervision and assistance to assigned individuals to ensure safety as specified in Individual Habilitation Plans (IHPs).
  • Assist individuals with daily living activities, including personal hygiene, grooming, money management, meal preparation, and socialization.
  • Implement training and behavioral programs as outlined in IHPs and maintain knowledge of each client's needs.
  • Provide transportation for individuals to medical appointments, recreational activities, and job attendance.
  • Assist individuals in meeting dietary needs by implementing dietary interventions specified by physicians and registered dieticians.
  • Supervise individuals in self-administration of medication and notify personnel when medication needs to be re-ordered.
  • Document the delivery of services accurately and timely as outlined in IHPs and company policy.
  • Report facility maintenance issues to ensure a safe living environment.
  • Perform other duties as assigned.

Requirements

  • Must be 18 years of age as required by the State of Texas.
  • Must have a high school diploma or equivalent.
  • Must have a current and valid Texas Drivers License.
  • Six months experience working with individuals with intellectual and developmental disabilities or related field.
