About the position

The Direct Service Coordinator (DSC) at Empowering Minds Resource Center plays a crucial role in assisting clients with mental illness to achieve greater independence and community integration. This position involves advocating for clients, helping them access and coordinate various benefits and services, and providing individualized support both on-site and in the community. The DSC employs a wellness-based approach to enhance clients' daily living skills and coping strategies.

Responsibilities

• Assist clients with their individualized rehabilitation plan goals in areas such as self-care skills, social skills, independent living skills, and health promotion.
• Monitor each client during in-person visits and document any changes in their emotional, psychological, or physical health.
• Document all client contact, interactions, interventions, and goal achievements monthly.
• Work cooperatively with family and other professionals to coordinate services with other agencies.
• Research and provide clients with community-based resources to meet their needs.
• Facilitate client participation in weekly group meetings to encourage socialization and coping strategy development.

Requirements

• Bachelor's degree in a human services field preferred.
• One year or more of experience working with children and families.
• Excellent written and oral communication skills.
• Strong time-management and organizational skills.
• Ability to work independently and as part of a team.
• Reliable transportation and a valid driver's license.
