About the position

As a Community Habilitation Specialist, you will play a crucial role in supporting individuals with developmental and learning disabilities. Your primary responsibility will be to provide direct services that enable these individuals to live independently and thrive within their communities. This position is available in various capacities, including full-time, part-time, and per diem, across Erie County. You will engage in community integration activities, assist with community travel and transportation, and help individuals manage their finances through budgeting and money management. Additionally, you will facilitate appropriate social interactions and promote healthy lifestyle choices, including exercise and nutrition. You will also support individuals in understanding and following medical instructions, attending medical appointments, and accessing necessary services to maintain their independence. In this role, you will work closely with an interdisciplinary team and provide one-on-one support to individuals receiving services. Your ability to communicate effectively, both verbally and in writing, will be essential in fostering positive relationships and ensuring that the needs of the individuals are met. You will be expected to utilize behavioral techniques and have a solid understanding of developmental disabilities to assist in the care of the individuals you support. Successful completion of First Aid, CPR, and Defensive Driving training is required upon hire, ensuring that you are equipped to handle emergencies and provide safe transportation for the individuals in your care.

Responsibilities

Requirements

  • High school diploma or equivalent required.
  • Valid New York State Driver's License in good standing.
  • Ability to work one-on-one with individuals receiving services.
  • Moderate level of written and oral communication skills.
  • High level of interpersonal skills.
  • Knowledge of behavioral techniques and developmental disabilities.
  • Ability to utilize various software programs.

Nice-to-haves

  • One year of relevant experience in health and human services or working with individuals with disabilities.
