About the position

The Director of Care Management at Cottage Health is responsible for overseeing all clinical, business, safety, quality, and regulatory aspects of inpatient and outpatient care management, social work, and community case management. This role involves partnering with the Population Health program to foster sustainable patient connections with community resources, promoting healing and overall health.

Responsibilities

• Oversee and direct all clinical, business, safety, quality, and regulatory dimensions of care management.
• Partner with the Population Health program to develop sustainable patient connections with community resources.
• Ensure compliance with clinical documentation integrity and government regulations.
• Facilitate team collaboration and data analysis for quality management.
• Generate reports and analyze data related to care management and social work.

Requirements

• Graduate of an accredited school of Nursing or Social Work with current RN or LCSW licensure in California or eligible.
• Current RN or LCSW license is required.
• Accredited Case Management (ACM) or Case Management Certification (CMS/CCM) to be obtained within 6 months of hire.
• Strong analytical abilities and intermediate proficiency in Word, Excel, and PowerPoint.
• Extensive knowledge of case management and social work concepts, clinical documentation integrity, and payer reimbursement.

Nice-to-haves

• Master’s Degree in a relevant field.
• Knowledge of Accountable Care Organizations and Bundled Payment Systems.
• Familiarity with Epic and Quality database.
