About the position

The Director of the Critical Time Transition Program for Children, Youth and Families (CTTP) is responsible for leading and directing a team that facilitates the safe transition of children and youth from hospital settings to community care. This role emphasizes a trauma-informed approach, prioritizing family support and skill-building within a strengths-based, relationship-focused culture. The Director will oversee program development, manage budgets, and ensure compliance with regulations while fostering a culture of diversity, equity, inclusion, and belonging.

Responsibilities

  • Lead the design, development, and implementation of the CTTP program in alignment with state regulations and agency policies.
  • Collaborate with CTTP programs statewide to provide input to OMH on program evolution.
  • Oversee all components of the CTTP program, including Critical Time Intervention (CTI) and Transitional Residential Setting (TRS) teams.
  • Manage complex program budgets and coordinate funding through various payers.
  • Ensure coordination of child/youth discharge to the community and facilitate warm hand-offs to community care management.
  • Build a high-performing team, prioritizing hiring from prevalent cultural groups and those with lived experience in the mental health system.
  • Monitor and respond to child/youth movement, including referrals, admissions, and discharges.
  • Provide clinical direction and oversight to all clinical aspects of the program.
  • Ensure continuous quality improvement of services and monitor program outcomes.
  • Participate in agency committees, projects, and initiatives as assigned.

Requirements

  • Master's degree required.
  • Minimum 5 years of relevant clinical experience post-Master's degree required.
  • Minimum 5 years of leadership or management experience required.
  • Experience with care coordination and/or hospital and community behavioral health system navigation required.
  • Active, unrestricted Mental Health Practitioner license in NYS required for one of the following: LMSW, LCSW, LMFT, Licensed Psychologist, LMHC, or Nurse Practitioner in Psychiatric Nursing with DNP.
  • Unrestricted, valid NYS driver's license for a minimum of 1 year with a clean driving record.

Nice-to-haves

  • CPR training within 60 days of hire and recertification annually.
  • Ability to obtain and maintain Transport for Evaluation (9.58) certification.
