About the position

The Director of Admissions (Clinical Liaison) at The Gardens at York Terrace is a full-time role focused on enhancing patient referral efforts and driving admissions within a skilled nursing facility. This position involves building and maintaining relationships with local hospitals, physicians, and the community, ensuring a seamless admission process for new residents.

Responsibilities

• Develop and nurture referral opportunities with hospitals, physicians, and other sources.
• Serve as the primary liaison between the facility and referral sources.
• Convert referrals into new residents through effective communication and coordination.
• Engage in community outreach to strengthen partnerships and awareness of our services.
• Collaborate with the facility team to ensure seamless admission processes.
• Ensure all admission documentation is accurate and completed in a timely manner.

Requirements

• Proven experience in healthcare sales, marketing, referral management, and/or admissions.
• Must hold a valid clinical license (RN, LPN, or Licensed Social Worker).
• In-depth knowledge of the long-term care (LTC) admission process is required.
• Strong understanding of insurance and reimbursement policies.
