About the position

The Director of Clinical Operations (DCO) at Personalized Health Partners is responsible for overseeing the delivery of high-quality primary care in a person-centered care model, particularly for seniors and residents in various healthcare settings. This role involves managing clinical operations, staff, and resources, ensuring compliance with regulations, and fostering relationships with key stakeholders. The DCO will spend a significant portion of their time in the field, providing oversight and support to clinical teams while also handling administrative responsibilities.

Responsibilities

  • Ensure the delivery of high-quality patient care and compliance with regulations and standards.
  • Provide ongoing performance feedback throughout the year and coach employees in disciplinary matters.
  • Identify educational needs and work with educators to develop and implement training.
  • Develop and implement operational strategies for the clinical team.
  • Review clinical opportunities and recommend strategic plans to improve them.
  • Maintain appropriate and compliant documentation within EMR software.
  • Develop relationships with key stakeholders within skilled nursing facilities (SNFs).
  • Support strategic initiatives regarding value-based care.
  • Provide an excellent customer experience to foster high customer satisfaction and retention.
  • Practice ethically and in accordance with the Scope and Standards of Practice of their profession and Board Certification.
  • Collaborate with the leadership team at facility DCOs and ensure adequate NP coverage to meet resident care needs.

Requirements

  • Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate.
  • National certification of AANP or ANCC.
  • Active, unrestricted state license/s.
  • Ability to obtain licensure in additional states as requested.
  • Active DEA license.
  • US work authorization.
  • 2+ years clinical experience as a Nurse or Nurse Practitioner in geriatric, adult, or family practice setting.
  • Ability to work in a flexible environment and schedule.
  • Passionate about providing healthcare to senior and low-income populations.

Nice-to-haves

  • 1+ years clinical management experience as a Nurse or Nurse Practitioner in geriatric, adult, or family practice setting.
  • Training or clinical experience in geriatrics.
  • 3+ years clinical experience as a Nurse Practitioner.
  • Experience with coding, ICD-10, and HCC (Hierarchical Condition Categories).
  • Experience or training working in a metrics, results-driven healthcare model.
  • Experience working in value-based care models.
  • Training or experience in geriatric, assisted living, and/or skilled nursing settings.
  • Experience working in a collaborative healthcare setting to drive positive outcomes.
