About the position

The Director of Quality and Population Health at Avenue360 Health and Wellness is responsible for leading the development and execution of the Quality Improvement (QI) Program, Quality Assurance and Performance Improvement (QAPI) Plan, and Population Health Program. This role focuses on enhancing the quality of healthcare delivery in Avenue 360 clinics, ensuring positive patient-centric outcomes, and providing direct patient care. The position requires collaboration with clinical leadership to implement strategies that improve health services and outcomes.

Responsibilities

  • Provide leadership in the development and execution of the Avenue 360 Quality Improvement (QI) Program and Population Health Program.
  • Participate as a key member of the health center clinical leadership team in developing and evaluating population health and quality strategies.
  • Oversee day-to-day operations and implementation of Quality and Population Health initiatives and policies.
  • Provide technical assistance, coaching, and training to staff in pursuit of the Quadruple Aim using QI theory and methods.
  • Apply population health concepts and implement quality improvement methodologies to ensure high standards of performance.
  • Collaborate with the EVP, Medical to oversee FTCA QA/QI compliance.
  • Enhance systems and tools for data collection, validation, analysis, and reporting on quality metrics.
  • Manage regular meetings and activities of the Avenue 360 QAPI committee under the Chief Medical Officer's direction.
  • Participate in the development and implementation of QI and healthcare policies, procedures, and protocols.
  • Provide direct outpatient clinical care for assigned patients and document care in the EMR.
  • Assure a safe care environment by instituting appropriate control measures.

Requirements

  • Master's degree in nursing, Health Care Administration, Informatics, or MBS preferred.
  • Current Registered Nurse license required.
  • Quality Management experience required.
  • Familiarity with ambulatory EMR systems required.
  • Experience with survey processes (e.g., Joint Commission, FTCA, HRSA, etc.) required.
  • Basic Life Support (BLS) Certification required.
  • Strong organizational and project management skills.
  • Excellent judgment and creative problem-solving skills.
  • Ability to work with minimal supervision in a fast-paced environment.

  • Knowledge of healthcare regulations and compliance standards.
