About the position
The Director of Social Services at HC&N Healthcare Solutions is a senior-level role responsible for overseeing the social work department within a nursing home. This position involves setting long-term and short-term goals for residents, addressing their social, emotional, and financial needs, and ensuring the delivery of high-quality social services programs tailored to the unique requirements of adult and geriatric residents.
Responsibilities
- Demonstrates knowledge of age-specific developmental factors for adult and geriatric residents in planning social services programs.
- Plans social services programs for residents with therapeutic, neurological, dementia, dialysis, IV therapy, and infectious disease needs.
- Follows up on admission inquiries and evaluates the needs of prospective residents to determine appropriateness of facility placement.
Requirements
- Experience as a Director of Social Work in a large long-term care (LTC) setting is required.
- Possesses growth in the profession through community involvement and continuing education.
- Capable of performing the essential functions of the job, with or without reasonable accommodation.
