About the position

The Director of Social Services at Ayden Healthcare of Waterville is responsible for overseeing the social service department, ensuring the implementation of psycho-social programming for residents, and collaborating with interdisciplinary team members. This role involves planning, organizing, staffing, and managing the department's operations to meet the needs of the geriatric population effectively.

Responsibilities

• Plan, organize, and direct the social service department's operations.
• Implement group and one-to-one psycho-social programming for residents.
• Coordinate with interdisciplinary team members to enhance resident care.
• Schedule and conduct group and individual sessions for residents.
• Review and countersign monthly progress notes and quarterly assessments.
• Complete the Psycho-Social Section of the MDS form as required.
• Track resident discharge and readmission for reassessment needs.
• Communicate with Psycho-Social staff regarding program changes and goals.
• Attend various meetings including Care Plan and Performance Improvement meetings.
• Assist residents and families with room changes and adjustment issues.
• Coordinate family meetings and special programs for residents and families.

Requirements

• 1 year of experience in social work, preferably in a skilled nursing facility.
• Understanding of the psycho-social dynamics of the geriatric population.
• Strong communication skills with residents, family members, and staff.
• Good administrative, managerial, organizational, and planning skills.
• Compassion and understanding for the elderly.
• Knowledge of ODH regulations relevant to social services.
