About the position

The Director of Social Services is responsible for overseeing the provision of medically related social services to residents in a skilled nursing and rehabilitation facility. This role involves ensuring that residents achieve and maintain their highest practicable physical, mental, and psycho-social well-being, while adhering to professional standards and regulatory requirements. The Director supervises social workers, collaborates with the facility administrator, and allocates resources effectively to meet the needs of residents.

Responsibilities

  • Meet with residents and families during the admission process to review policies on advanced directives and resident rights.
  • Conduct and document social services evaluations to identify resident problems and needs.
  • Provide or arrange for social work services based on resident and family needs.
  • Assist residents with financial planning as needed.
  • Keep residents' families informed about resident issues, personal needs, and changes in care assignments.
  • Maintain a current list of community resources for referrals.
  • Establish criteria to ensure services meet established quality standards.
  • Demonstrate interpersonal skills to interact effectively with residents, families, and staff.
  • Evaluate residents' behavior and psycho-social needs to inform treatment interventions.
  • Implement social service interventions to achieve treatment goals and enhance residents' quality of life.
  • Communicate effectively in an interdisciplinary care setting.

Requirements

  • Bachelor's Degree in Social Work or a related human services field.
  • One year of supervised social work experience in a healthcare setting.
  • Current Social Work license in the state.
  • Strong organizational and analytical skills.
  • Ability to supervise and motivate others.
  • Excellent oral and written communication skills.
  • Knowledge of state and federal laws and regulations related to social work in long-term care.

Nice-to-haves

  • Experience in long-term care settings.
  • Familiarity with community resources for social services.
