About the position

The Director of Social Work at Covenant Living is responsible for overseeing the social service functions within a Continuing Care Retirement Community. This role involves planning, developing, and implementing programs to address the emotional and psychological needs of residents across various levels of care. The Director acts as a resident advocate, participates in assessments, and facilitates inter-facility transfers, ensuring compliance with state and federal regulations while maintaining a supportive environment for residents and their families.

Responsibilities

  • Direct the social service functions including planning, development, and implementation of programs for residents' emotional and psychological needs.
  • Participate in the resident assessment process and facilitate inter-facility transfers.
  • Serve as a resident advocate within the campus.
  • Assist prospective residents, families, and discharge planners for skilled nursing care needs.
  • Provide social assessments upon admission and identify psychosocial needs in compliance with regulations.
  • Assist with resident orientation to the skilled nursing facility and services.
  • Collect and record admission data including social, medical, and financial information.
  • Maintain documentation in residents' health records related to social services care components.
  • Participate in resident care planning meetings for communication and problem-solving.
  • Assist residents and families in selecting new physicians as necessary.
  • Communicate appropriate information regarding admissions or discharges to departments.
  • Function as an advocate for residents and families to ensure resident rights are upheld.
  • Provide regular contact with families and act as a liaison with the resident care team.
  • Establish family support meetings for medically related social and emotional needs.
  • Explain government and facility policies to new residents and families.

Requirements

  • Master's degree in Social Work preferred; Bachelor's degree in Social Work required.
  • License as a Social Worker in the State.
  • Four years of experience in a medically related field with long-term care experience.
  • Two years of supervisory experience in the social work field.
  • Knowledge of community resources.
  • Excellent verbal and written communication skills in English.
  • Good organizational skills with attention to detail and accuracy.
  • Ability to work with confidential information and establish trusting relationships.
