The Discharge Coordinator at Denver Springs plays a crucial role in the discharge planning process, ensuring effective communication with clients, families, and referral sources. This position is integral to providing resource information for future care needs and assisting in crisis situations to ensure patient safety. The role emphasizes a collaborative work environment focused on compassionate behavioral health services, including crisis stabilization and outpatient treatment.

Responsibilities

• Actively involved in the discharge planning process.
• Communicates with clients, families, and outside referral sources, demonstrating proactive communication with those involved in the patient's treatment.
• Provides resource information to patients and families as appropriate for future care needs.
• Effectively communicates to Utilization Review on treatment and Length of Stay issues.
• Demonstrates appropriate crisis intervention and de-escalation skills, acting as a team member when crises occur to ensure patient safety.

Requirements

• Must have at least one year of case management/discharge coordinator experience.
• High school degree or degree from an accredited college or university in social work, recreational therapy, or an appropriate therapeutic field preferred.
• Valid Driver's License and a favorable Motor Vehicle Report and proof of insurance when using own auto.
• CPR and CPI Certification within 30 days of employment.
• Completion of additional age-specific training within 30 days of employment as required. Must be at least 21 years of age.

Nice-to-haves

• Previous experience in a psychiatric health care facility, with direct experience working with chemical dependency, dual diagnosis, psychiatric, and geriatric patients preferred.
